   Dear,

   I want to make an appointment to obtain a medical certificate for my girlfriend Gerong Egelin, necessary for a visum for Belgium.
   The Belgian embassy needs this certificate.
   A medical certificate is required whenever an applicant has to prove that he is not carrying any disease that might endanger public health. The diseases in question in the medical certificate are listed at annex to the law of 15 December 1980 on entry, stay, settlement and removal of foreign nationals.
   The embassy gives me your clinic to make an appointment: Clinic of Dr. Peralta Unit 506, 5/F GC Corporate Plaza – 150 Legaspi St. – LEGASPI Village 1229 Makati City, Manila
